Peroperative Prediction of Acute Perforative and Gangrenous Appendicitis by Clinical Features of Patients



Abstract
650 patients who had proven pathologically to have acute appendicitis were studied retorspectively to assess clinical parameters leading to the preoperative prediction of perforative or gangrenous type. The age and sex, nature of pain,
accompanying
symptoms, physical findings, and labolatory findings were taken into account for variables and analyzed statistically. As results, when patient was over 40 years of age, when the duration of pain was more than 48 hours and the site of pain was
localized
to RLQ, when body temperature was above 37.6¡É, pluse rate was more than 90/min, and diffuse tenderness and/or rebound tenderness was present, and when leukocyte count is over 14000/§§ on CBD and hematuria, pyuria, proteinuria was present on
urinalysis,
especially when these found simultaneously the perforative or gangrenous appendicitis was so probable that adequate treatment plan must be made accordingly for successful results.
